Chronic Disease Management

Health Wise program aims to assist people in making lifestyle changes that will help them to better manage their long term health condition.

Key Workers are trained in self-management and health coaching and support people in the changes they would like to make to help them get the most out of life.

On the first appointment the Key Worker will talk to you about what is important to you and your health. Together, a “Healthy Living Plan” will be made and some goals set for the changes you would like to make. Follow-up appointments with the Key Worker will be made as needed.

The “Healthy Living Plan” may include access to services such as counselling, diabetes education, dietetics, exercise, occupational therapy, physiotherapy, podiatry, nursing and better health self-management and smoking cessation program. A report will be sent to the referring doctor.

Eligibility Criteria: Suitable for people with diabetes, heart failure, osteoarthritis or chronic lung conditions (asthma and COPD) who need assistance in managing their long-term condition.
Fees: Fees are set as per as per DCH Fees Policy. $8 per consultation for health care or concession card holders $12 per consultation for low income $25 per consultation for middle to high income Arrangements can be made if financial hardship exists.
Referral: Referrals can be Self referral, from General Practitioners and external health professionals.
