Introduction

The Chronic and Complex Care program targets clients with long term chronic conditions and those with complex needs. The teams include: General Allied Health, Hospital Admission Risk Program (HARP), Health Wise, Living Well and Planned Activity Groups (PAG).

The teams operate in a very client-focused way to link in other disciplines within DCH to better serve the health needs of our community.
