Community Liaison
The Community Liaison team is located within the Health Promotion Unit at DCH.
It’s primary function is to liaise with the local community and to enhance participation in all aspects of DCH, including service planning, usage and evaluation; volunteering; self support groups and management. It also works to promote social connectedness and foster good mental health
Eligibility Criteria:
Consistent with DCH criteria i.e the commuity within City of Darebin, with priority given to people experiencing the highest level of disadvantage, discrimination and poor health.
Specific eligibility criteria exist for some of the Self Support Groups.
Fees: In line with the DCH Fees policy a gold coin donation per session attended is expected from group members. In addition, some groups charges a small fee - this is usually associated with costs for running the groups and / or outings.
Referral:
No referral is necessary for group members.
How to Make an Appointment: Not applicable
Waiting Time: Primarily within standard DCH opening hours
Location of Service:
Across all sites, although majority of activities/groups occur at East Reservoir and Northcote sites
Hours Of Operation: Primarily within standard DCH opening hours
