Dietetic services:
Dietitians are food experts that can provide you with information on all aspects of food and nutrition. Dietitians support individuals and groups in the community to make healthy food choices.
Diabetes Education Services:
Diabetes educators can help people with diabetes to manage their condition and live a healthier, more productive life with diabetes.
Eligibility Criteria:
As per DCH policy.
A number of priority groups have been identifies for high priority access to primary care services by DCH. These include:
Aboriginal and Torres Straight Islanders
Newly arrived refugees and Asylum Seekers
Children under 12 years old and their families
People who are socioeconomically disadvantaged
Fees: Fees are set as per as per DCH Fees Policy.
$8 per consultation for health care or concession card holders
$12 per consultation for low income
$25 per consultation for middle to high income
Arrangements can be made if financial hardship exists
Referral:
People can access the dietetic service via
Self referral
Referral by family members/carers
Internal Referrals
External Referrals from local agencies and services.
How to Make an Appointment: Contact the Client Access and Referral Service on 8470 1111 at the East Reservoir site to arrange an appointment for the appropriate service and location.
Waiting Time: Diabetes education 2-3 weeks
Dietetics 3-5 weeks
Interpreters: Available as required
